I would like your input as to how you handle short forms for variations of:
"Sponge, needle, and instrument counts correct."
There are so many variations!

Here's a start: sanc = sponge and needle counts sancwc = sponge and needle counts were correct sancwcx = sponge and needle counts were correct times 3 sancwr = sponge and needle counts were reported sancwrc = sponge and needle counts were reported correct sancwrcx = sponge and needle counts were reported correct times 3 snai = sponge, needle, and instrument snaic = sponge, needle, and instrument counts snaicwac = sponge, needle, and instrument counts were all correct snaicwc = sponge, needle, and instrument counts were correct snaicwcx = sponge, needle, and instrument counts were correct times 3 I'd love to see some more entries. Anybody have ideas? I'm sure Harrie's Productivity Talk forum has this info. Problem is, the search function isn't currently enabled or working there [last time I looked, 2 days ago], so finding the info can be a bit tricky. Boy, Debbie, if you aren't right, there are so many variations! This is a great subject. This one drives me crazy, too! I still don't have enough of the variations I hear.
I'll add a few to what's already been posted:
lap, sponge, and needle count lap, sponge, and needle counts lap, sponge, and needle counts were correct all lap counts and sponge counts were correct lap, instrument, and needle counts instrument count, sponge count, and needle count was correct needle, lap, and sponge counts were correct at the end of the case x2
